Ok, a buddy of mine was trying to ID this. I know this and probably have it....but I can't place it. I'm about 99% sure this is on either a KPM or Amphonic LP. Someone here should be able to give a positive ID on this (then i'll kick myself later for not figuring it out myself lol) :
